Emergency Treatment
If your work-related injury or illness requires immediate medical attention, you should proceed to the nearest emergency treatment facility. “Emergency care means health care services for a medical condition displaying itself by acute symptoms of enough severity such that the lack of immediate medical attention could reasonably be expected to place your health in serious danger.” If further treatment is required, you must follow the standard MPN procedures by selecting a primary doctor from the list of MPN providers. You may contact your Supervisor or REM, LTD within 3 business days following the injury, emergency admission, service or procedure for assistance in identifying an appropriate MPN provider.
Selecting a Participating Medical Care Provider
As a patient in the MPN, you have the right to see a doctor close to your home or workplace within 3 business days for non-emergency care or within 20 business days of receiving a referral to a specialist. Due to the extensive coverage of the MPN, you should not have to travel more than 15 miles or 30 minutes to see your treating provider, or 30 miles or 60 minutes to see a specialist.
In the MPN Plan, you will select a primary treating provider from within the MPN. In the event that you need to be seen by a specialist, you may discuss it with your Intracorp case manager or your primary treating provider to ensure seeing a specialist within the MPN who is in a specialty appropriate to treat your injury or condition. You may choose a specialist or ask your treating doctor for assistance in finding a specialist. If there is not a network specialist within 30 miles of your home or workplace, you may be asked to travel to a suitable in-network specialist within a reasonable distance or you may be allowed to go outside the network. Please contact REM, LTD for assistance in selecting a suitable specialist.
Predesignation
You may predesignate your primary care physician to treat you for a work-related injury, prior to injury occurrence. This physician must have previously treated you and he or she needs to agree to be predesignated. If your physician does not agree to participate, you will be required to seek care from an MPN provider. REM, LTD can provide a predesignation form.

預先指定
