The Programme

The TeLeCare (TLC) service is a new care management initiative for patients with well-controlled conditions:

电话诊疗（TLC）服务作为一项全新的保健管理举措，专为患有以下疾病但控制良好的病患推出：

Stroke
• Chronic kidney disease
• Peripheral vascular disease
• Epilepsy
• Hypothyroidism

中风 
• 慢性肾脏病 
• 周边血管疾病 
• 癫痫症 
• 甲状腺机能减退症

Diabetes
• Hypertension
• High cholesterol
• Asthma
• Ischaemic heart disease

糖尿病 
• 高血压 
• 高胆固醇 
• 哮喘 
• 缺血性心脏病

Our trained senior nurses will review your medical condition over phone.
If your health condition is stable, you will continue your medication till your next review date. Our nurses will arrange for an earlier doctor’s review if any health concern is detected during the phone review.

我们训练有素的高级护理人员将以电话方式检查您的身体状况。 
如果您的健康状况稳定，那么在下次检查日期之前，您都将继续接受当前的药物治疗。 
如果通过电话诊疗服务发现您健康情况不稳定，则我们的护士将安排医生进行提前为您检查。
